ACCEPTANCE OF NEW PLACEMENT FORMS NEEDED:
Upon acceptance of a new placement, these are the forms you will need to complete: (click on form name to access the form)
- 3 Day Medical Screening Form (The 3 day medical screen, must be completed within 3 days of accepting placement)
- 30 Day Medical Screening Form (This appointment should be scheduled at the time of placement with a Primary Care Physician and completed within 30 days of the child being placed in the home)
- Clothing Inventory (To be completed and given to the caseworker with the 3 day medical form)
- Dental Exam Document (To be scheduled at time of placement and completed within 6 months of accepting placement unless otherwise instructed by ECDJFS.
- Optical Exam Document (To be scheduled at time of placement and completed within 6 months of accepting placement unless otherwise instructed by ECDJFS.
MONTHLY FOSTER PARENT FORMS:
- Critical Incident Report (To be completed if child in your care is injured resulting in a mark or medical attention)
- Invoice Form
- Monthly Invoice Form
- PROGRESS REPORT — updated 2-24-20
- PRESCRIPTION LOG updated 2-24-20
- Annual Medical Screening Form
Annual Review Documents:
These forms will be mailed to you at a minimum of 120 days prior to the Annual Review date of your license: (Please complete them and return them to your licensing specialist at a minimum of 30 days prior to the due date)

New Member to the Household Foster/Adopt:
- Change in Household Occupancy, Marital Status, or Address (Requirement to provide notice of these changes immediately to ECDJFS)
- Medical form JFS1653 (Required for new household member – Last physical is required for foster child being adopted)
- Local Criminal Background Release/Child Welfare History Release (a criminal records check shall be initiated within ten working days of the date the person becomes a household member)
- Juvenile Background Release
- FBI and BCI&I Fingerprints (Required for new household member over the age of 18)
